If it was just a normal tablet pc running windows and has a usb port, it'd probably be relatively easy to do something like that.  Joystick, couple of buttons and an ipac.

Given that previous thinkgeek april fool's jokes have turned out to be true, anything is possible, but probably never with an ipad.

I'd probably skip the joystick and go for the built in leads on an ultrastik, but that's just personal preference.  Maybe I'll try building something like that for my tablet pc...
