I had a few spare minutes last night, so I was able to unfold and setup DM (in my vacant T2 spot).  Aferwards I was going through all the goodie bags that originally came with the machine, and it looks like this game was purchased new from Starburst back in the day.  (I found the sales guy's business card in the coin box)

I'll snap a few more pics and post tonight.
