Two years ago I bought a second clinic with my partener, we want to see if we can have 2, 3, 4, etc. The second one is for sell right now. Life is too short for me to have big ambition and no more time...
So If you have enought money to live and be happy right now, take care with your decision.
I also realized that I'm one of the lucky to have the kind of job I have, If I want to do more, i just have to work more. Most people I know have to work more for the same money!

Take the straight salary raise and stay away from the commission based job. The economy is sucking. Last thing you want is to have your income based on you making sales in a bad economy. Your better with 18% guaranteed of something than looking at sales to give you a potential 7% more, particularly in a bad economy. Commission based you could equally end up with a load of post holes.
Just to be sure I would investigate what happend to your predecessor in that sales position. If he got promoted out of it in a bad economy and now is the King of England maybe not so bad. If he hit the road then there is likely a reason that you may also encounter.

I'd take the commission; I have been commission my whole life and if I want more money I just work more. It took a while to get used to swings in pay but overall I have made way more than a straight salary position. If you work more on a salary job your effective wage goes down. More work = same pay on a salary job. More Work = more money on a commission job.
At any rate you will be making more and I can tell you that if the economy weakens more the government will put money towards infrastructure and your business will not be down for long.
